Job Description

BASIC FUNCTION

Conduct Mobile Outreach activities via a field-based model that combines outreach, HIV/HCV/STI testing and Evidence Based interventions (Think Twice, Motivational Interviewing) with recovery support services (case management and linkage to care), and social marketing campaigns.  The population of focus: male and female racial/ethnic minority individuals living with or at risk for HIV/AIDS ages 18 and older, with a special focus on racial/ethnic minority YMSM (ages 18-29), MSM (ages 30 and older), and gay, bisexual, transgender individuals, returning veterans, and population of focus' needle sharing and/or sexual partners. 

 

SPECIFIC RESPONSIBILITIES: Conduct both HIV/HCV/STI Testing. Conduct confirmatory test for all preliminary HIV positives and actively link all HCV/STI positives/reactives for confirmatory testing/treatment. 

  1. Conduct outreach in specific targeted areas with a high incidence of Substance Abuse, HIV and viral Hepatitis and capture client demographics and risk factors. For all encounters eligible for programmatic services, provide on-the-spot services
  2. Enroll individuals who have substance use disorder/co-occurring substance use and mental disorder who are at high risk or who are HIV positive into the Linkage to Care program
  3. Provide comprehensive case management to individuals who have substance use disorder/co-occurring substance use and mental disorder who are at high risk or who are HIV positive.
  4. Distribute condoms and education information materials
  5. Actively refer consumers to viral Hepatitis B testing; actively refer consumers to Hepatitis A and B vaccinations.
  6. Conduct Substance Abuse field-based screening, assessment, fast track intake and linkage to treatment.
  7. Conduct the Think Twice behavioral intervention
  8. Oversee the collection of GPRA data ensuring that it is in accordance with funding guidelines. Ensure that all necessary information is being captured through client interviews. Conduct required follow-up and discharge paperwork with GPRA clients.
  9. Ensure that all objectives, goals, and outcomes projected in the workplan are being tracked and met.
  10. Attend bi-weekly supervisions, monthly team meetings, monthly department meetings, monthly Outreach Committee meetings, and quarterly tester in-service meetings, and mandatory staff meetings.
  11. Other duties as assigned.

 

QUALIFICATIONS: High School Diploma or GED required/Bachelors preferred.  One (1) year experience in conducting outreach. One (1) year Substance Abuse, Mental Health, and or HIV/AIDS and viral Hepatitis experience preferred. Extensive knowledge of targeted communities where outreach will be conducted.
