Provide Long Island's YMSM (Young Men who have Sex with Men) of color and their sexual partners with comprehensive HIV prevention. Reduce HIV infections, increase access to care, increase the individuals who are reengaged into care, and increase viral suppression. Increase the number of High-risk HIV-negative individuals who are prescribed PrEP and who are linked to High-Impact HIV Prevention behavioral interventions.


  1. Conduct outreach and promotion in specific targeted areas where there is a high incidence of YMSM and HIV (e.g. local gay bars/clubs, pride organizations, Fire Island, social medial, etc.). Additionally, attend health fairs and community events to promote the program.
  2. Develop and maintain a Youth Advisory Board (YAB) which will meet quarterly.
  3. Develop and maintain a Mentoring Program. Follow the 5 stages: Recruitment, Screening, Enrollment, Program Involvement, and Closure.
  4. Mentor 10 individuals annually and provide them with tools and resources to become strong leaders within their community.
  5. Refer and link newly diagnosed HIV-positive individuals and previously diagnosed HIV-positive individuals who have fallen out of care to health care providers/infectious disease professionals. Additionally, link HCV and Syphilis preliminary positive individuals to confirmatory testing and link individuals who tested positive for Chlamydia and/or Gonorrhea to treatment.
  6. Link clients to medication adherence services and Partner Services.
  7. Enroll clients into the CLEAR HIP behavioral intervention. The Care Navigator will refer clients to Healthy Relationships and Mpowerment.
  8. Refer PLWHA to support services (insurance navigation and enrollment, mental health counseling, substance abuse treatment, housing services, transportation services, employment services, and sex education, etc.).
  9. Refer High-risk Negative individuals to PrEP and/or nPEP.
  10. Enroll clients into the VOICES/VOCES behavioral intervention.
  11. Enroll clients into the Linkage to Care Program providing linkages and referral to medical care, HIP behavioral interventions, PrEP/nPEP services, and other essential and supportive services.
  12. Refer individuals who are at very high-risk for HIV support services (Medication Adherence, Substance Abuse Treatment, HIV Primary Care, Mental/Behavioral Health Services, Housing Services, Prevention Education Services, Case Management, Screening/Testing for TB or Syphilis, PrEP and nPEP, etc.).
  13. Distribute condoms to YMSM of color or who living with HIV/AIDS or who are High-risk Negative.
  14. Ensure that outcomes projected in the work plan are being tracked and met.
  15. Generate monthly, quarterly, and annual reports.
  16. Input programmatic data into the AIDS Institute Reporting System (AIRS).
  17. Other duties as assigned.



High School Diploma or GED required.  Bachelor's degree preferred.  One (1) year experience in conducting outreach. One (1) year HIV/AIDS experience preferred. Extensive knowledge of the targeted community and where outreach will be conducted is preferred. Familiarity with the cultural attributes of the target population, related HIV risks and harm reduction strategies are required.  Good verbal and organizational skills a must. Spanish language skills preferred.
