Hauppauge, NY Full-time Position






To assist the Care Coordination team in the outreach and enrollment process of providing Care Coordination services to clients.



  1. Conducts intensive outreach efforts to persons who have been identified as Health Homes eligible by way of phone calls, written correspondence and unscheduled appointments.
  2. Provides and discusses information on services to potential clients of Health Home Program and assist in the delivery of services upon enrollment.


  1. Assist in the development of the Hot Line Intake, Agency and Health Home consents.


  1. Assist in making phone calls and visits to clients, other agencies and providers in order to obtain information pertinent to the assessment, service plan development, and delivery of services.


  1. Assist and advocate in the coordination of medical/behavioral care, attend case conference meetings with other service providers and participate as needed.


  1. Assist and advocate in the process of maintaining active Medicaid & entitlements provided through Department of Social Services, Social Security Administration and other community service providers.


  1. Distributes informational material to providers that describe what program services are available to eligible clients to further develop a referral base. Conduct outreach informational programs where accepted.


  1. Assists Care Coordinators with service provision and follow-up on cases, as required and according to client need.


  1. Ensure documentation of Outreach activities are in appropriate data base per client's assigned enrollment and is in accordance with the Lead Health Home's Departmental standards.


  1. Communicate all information relative to client's progress with the Care Coordinator.


  1. Attend required training as assigned.



  1. Assure client information is kept confidential at all times.


  1. Assist the Health Homes Services Coordinator in the monitoring of client's Medicaid eligibility.


  1. Other duties as assigned.




Reports to the Health Homes Services Coordinator




High school diploma or equivalent and two years of experience relative to the delivery of human services required.
